Patient Reach 360, LLC (“PR360,” “we,” “us,” or “our”) respects the privacy of individuals who use our consumer-facing websites, landing pages, assessments, forms, scheduling tools, communications, and related online services (collectively, the “Sites” and “Services”). Because PR360 operates multiple consumer-facing Sites, this Privacy Policy applies to Sites that link to or display this Policy rather than listing every domain individually.
This Policy explains what information we collect, how we use and disclose it, the technologies and service providers that may process it, and the choices available to you.

First and last name.
Telephone number.
Email address.
ZIP code.
Insurance provider.
The type of healthcare service or provider you are seeking.
Appointment preferences, scheduling information, and communications with PR360 or participating Healthcare Providers.
Any other information you voluntarily provide through a Site, form, telephone call, text message, or email.
PR360 does not currently request clinical medical records, diagnoses, medication lists, laboratory results, or detailed treatment records through its standard consumer lead form. However, information about the healthcare service you are seeking, when combined with identifying information, may be sensitive or health-related information under applicable law.
Information Collected Automatically
IP address, browser type, device type, operating system, and device identifiers.
Pages viewed, links clicked, referring URLs, timestamps, and interactions with the Sites.
Cookie, pixel, tag, advertising, attribution, and analytics identifiers.
Approximate location inferred from IP address or information you provide.
Communications metadata and, where permitted by law, records relating to calls, texts, or emails.
How We Use Information
To respond to your request and provide the Services.
To identify a participating Healthcare Provider based on factors such as geographic area, the service sought, and the insurance provider you identify.
To transmit your request to an appropriate participating Healthcare Provider.
To facilitate appointment scheduling, rescheduling, reminders, and follow-up.
To communicate with you by telephone, text message, and email.
To operate, secure, troubleshoot, maintain, and improve the Sites and proprietary PR360 software.
To measure lead quality, conversion, scheduling, campaign performance, and service effectiveness.
To perform analytics, advertising attribution, and campaign measurement.
Where permitted by law and with any consent required by law, to market PR360 services, participating providers, or health-related opportunities.
To prevent fraud, abuse, security incidents, and unlawful activity.
To comply with legal obligations, enforce agreements, and establish or defend legal claims.
How Provider Matching Works
You may submit information without knowing which Healthcare Provider will receive your request. PR360 may share your information with a participating Healthcare Provider that, based on information reasonably available to PR360, appears to serve your geographic area, offer the type of service you are seeking, and potentially accept the insurance provider you identify.
PR360 does not determine insurance eligibility, benefits, network status, prior authorization, medical necessity, or treatment eligibility. Those determinations are made by the Healthcare Provider and/or your insurer.
How We Disclose Information
PR360 may disclose information in the following circumstances:
Healthcare Providers. We may share your information with one or more participating Healthcare Providers as necessary to respond to your request, determine whether the provider can assist you, facilitate contact, or schedule an appointment.
Service Providers and Technology Vendors. We use third parties to support hosting, infrastructure, security, communications, analytics, advertising attribution, customer relationship management, website operation, scheduling, and related functions. Current categories and platforms may include Fly.io, Cloudflare, Google, Meta, Stripe, LeadConnector/GoHighLevel, email and SMS providers, website hosting/building providers, and other vendors supporting PR360’s proprietary lead-generation, qualification, scheduling, and analytics software. These vendors may change over time.
Business and Professional Advisors. We may disclose information to attorneys, accountants, auditors, insurers, consultants, and other professional advisors where reasonably necessary.
Legal and Safety Purposes. We may disclose information when reasonably necessary to comply with law, subpoena, court order, legal process, regulatory request, protect rights or safety, investigate fraud or security incidents, or enforce our agreements.
Business Transactions. Information may be disclosed in connection with an actual or proposed merger, acquisition, financing, reorganization, sale of assets, bankruptcy, or similar transaction, subject to applicable law.
PR360 does not sell consumer contact information to unrelated third parties for their independent use in exchange for money. Certain state privacy laws may define “sale,” “sharing,” or “targeted advertising” more broadly and may treat some advertising or analytics disclosures as a sale or sharing even when no money changes hands. Where such laws apply, PR360 will provide required notices and choices.
Cookies, Pixels, Analytics, Advertising, and Attribution
PR360 and its vendors may use cookies, pixels, tags, APIs, local storage, device identifiers, and similar technologies to operate the Sites, maintain security, understand usage, measure campaign performance, attribute leads to advertising sources, and, where permitted, support advertising.
PR360 currently uses or may use services provided by companies such as Google and Meta and may use other analytics or advertising platforms. These technologies can receive information such as IP address, device/browser information, page activity, advertising identifiers, and campaign or referral information.
Because PR360’s Sites relate to healthcare services, PR360 seeks to limit the transmission of directly identifying form-field content and sensitive health-related information to advertising and analytics vendors. Tracking technologies should be configured and reviewed so that information submitted in lead forms, appointment requests, or other sensitive fields is not transmitted to advertising platforms except where legally permitted and appropriately disclosed or consented to.
Depending on applicable law and the technology used, you may be offered cookie controls or other mechanisms to opt out of non-essential advertising or analytics technologies. Browser-level controls may also limit cookies, but disabling cookies can affect Site functionality.
Communications and Marketing
PR360 and participating Healthcare Providers may use the contact information you provide to respond to your request and communicate about provider matching, appointments, reminders, follow-up, and related services.
Where permitted by law and with any consent required by law, PR360 may also send marketing communications regarding PR360 services, participating providers, or health-related opportunities. Marketing consent is not a condition of obtaining healthcare services. You may opt out of marketing emails using the unsubscribe link and opt out of marketing text messages by replying STOP. Opting out of marketing does not necessarily stop non-marketing communications concerning a request, appointment, security issue, or transaction where such communications are permitted by law.
HIPAA and Other Health-Privacy Laws
PR360 is not itself a healthcare provider or health plan merely because it operates the Sites or receives health-related consumer inquiries. HIPAA applies to covered entities and to business associates when they create, receive, maintain, or transmit protected health information on behalf of a covered entity for a covered function or service.
PR360’s status under HIPAA may depend on the facts of a particular relationship, data flow, and service arrangement. If PR360 performs functions for a HIPAA-covered Healthcare Provider that make PR360 a business associate, PR360 and the Healthcare Provider should enter into an appropriate Business Associate Agreement and PR360 will handle applicable protected health information in accordance with HIPAA and that agreement.
Information that is not protected by HIPAA may nevertheless be protected by other federal or state privacy, consumer-protection, health-data, breach-notification, or communications laws. PR360 will handle such information in accordance with applicable law and this Privacy Policy.
Security
PR360 uses administrative, technical, and physical safeguards designed to protect personal information appropriate to the nature of the information and the Services. These measures may include access controls, authentication, encryption in transit where appropriate, vendor controls, logging, monitoring, and other security practices.
No method of transmission or storage is completely secure. PR360 cannot guarantee absolute security, and users should use reasonable care when transmitting information electronically.
Data Retention
PR360 retains personal information for as long as reasonably necessary to provide and improve the Services, maintain business and provider records, measure lead and campaign performance, resolve disputes, enforce agreements, prevent fraud and abuse, comply with legal or contractual obligations, and maintain appropriate backup and security records.
Retention periods may vary by data type and purpose. PR360 may retain certain information after a deletion request where retention is reasonably necessary or permitted by law, including for legal compliance, security, fraud prevention, backups, recordkeeping, dispute resolution, or to document and honor opt-out requests. PR360 should maintain an internal retention schedule and periodically review whether retained data remains necessary.
Your Privacy Choices and Requests
Depending on where you reside and applicable law, you may have rights to request access to, correction of, deletion of, or information about certain personal information, or to opt out of certain advertising, sale, sharing, or profiling activities.
You may submit a privacy request to info@patientreach360.com. PR360 may need to verify your identity before completing a request. PR360 may deny or limit a request where permitted or required by law and will explain the basis when legally required.
PR360 does not promise that every copy of information can be immediately erased from backups, archived systems, legal records, security logs, or records that PR360 is legally permitted or required to retain. Where appropriate, information may instead be isolated from ordinary business use until deletion occurs through normal retention cycles.
Children
The Sites and Services are intended for adults age 18 and older. PR360 does not knowingly solicit personal information from children under 18 through its consumer lead-generation Services. If PR360 learns that it collected information from a child in a manner inconsistent with applicable law, it will take appropriate steps to delete or otherwise address the information.
Third-Party Websites and Healthcare Providers
Healthcare Providers and third-party websites, platforms, or services have their own privacy practices. Once a Healthcare Provider receives your information, the provider’s use of that information may be governed by its own privacy notice, Notice of Privacy Practices, consent forms, and applicable law. PR360 is not responsible for the independent privacy practices of Healthcare Providers or unrelated third parties.
Data Breach and Incident Response
PR360 maintains processes intended to identify, investigate, contain, and respond to security incidents. If an incident involving personal information requires notice under applicable law, PR360 will provide notifications to affected individuals, regulators, Healthcare Providers, or others as required by the applicable law and circumstances.
Changes to this Privacy Policy
PR360 may update this Privacy Policy from time to time. The “Last Updated” date will identify the current version. If required by law, PR360 will provide additional notice or obtain consent before materially changing how previously collected information is used or disclosed.
